Qualifying conditions

Qualifications for a Registration Certificate in New York State.

A patient must be diagnosed with at least one of the qualifying conditions to received MMJ card:

Cancer
HIV or AIDS
Amyotrophic lateral sclerosis ALS ( aka Lou Gehrig’s Disease)
Parkinson’s disease
Multiple sclerosis
Spinal cord injury with spasticity
Epilepsy
Inflammatory bowel disease
Neuropathy, Radiculopathy, sciatica, pinched nerve, numbness, tingling
Huntington’s disease
Post-Traumatic stress disorder P.T.S.D. (Anxiety, Depression, Insomnia, Panic Attacks, etc)
Chronic pain ( Arthritis, Migraine, Fibromyalgia, Back Pain, Neck Pain, Severe Dysmenorrhea, Endometriosis, Uterine Fibroids, etc)
Opioid Use Disorder/Substance Use Disorder, you must be enrolled in an Article 32 Treatment program
The severe debilitating or life-threatening condition must also be accompanied by one or more of the following associated or complicating condition: cachexia or wasting syndrome, severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms, P.T.S.D or opioid use disorder

The Medical Documentation Necessary for Eligibility:

A copy of their medical records to confirm your diagnosis.

The recommended medical documentation should document your qualifying medical condition.

Please include your current medical diagnosis, list of medications, any surgeries, or diagnostic tests confirming your qualifying condition.

Patient must be a resident of the state of New York.

Provide a copy of your valid NYS Drivers License or Non-Driver ID

Proof of Identification & Residency:

You’ll need to provide one of the following items to show proof of residency:

New York State Driver’s License or New York State Non-Driver ID Card
Copy of a government-issued identification card that contains your name and New York State address.
